Before the “head in the door smashing scene” ends you see the Bride untying the elastic strings that keep Buck’s trousers up. Then when we next see her wheeling herself around in the car park she is now wearing Buck’s clothes with his name tag. She has obviously swapped clothes.

Well I think it is quite ironic and hilarious the fact that Elle, Buck and the Bride get away with being doctors.
Elle walks into the first hospital and to us (the audience) she looks very conspicuous in her normal clothes. However the other characters in the hospital do not seem to pay any attention to her. Then when she changes into her nurse outfit she looks ridiculously obvious (to the audience) especially with the red cross on her eye-patch. It cracked me up!
Then you have Buck, a sleezy guy who has tattoos and eyes up other nurses, oh and drives a pimp mobile. From the audience’s POV, it is hard to believe how he gets away with what he is doing. But again, the other characters in the hospital do not seem to notice.
So, when it comes to the Bride’s turn to escape, I think QT is saying the only way she will be able to escape is if she is in “disguise” regardless of the way she goes about doing it i.e. in a a wheelchair! It’s actually great humour by Tarantino. Props to him…
